Biomechanical Models of Reconstructed Middle Ear


Abstract eng:
The biomechanical models of the human middle ear under tympanoplasty for different variants of reconstruction of the ossicular chain are proposed in this paper. Static and dynamic problems, including evaluation of initial stresses caused by introducing a prosthesis replacing the “malleuseincus” or “malleuse-incus-stapes” chain as well as analysis of eigen-modes and eigen-frequencies of the reconstructed middle ear (RME), are studied. For both problems three variants of tympanic membrane (TM) reconstruction techniques [1], a) cartilage plate, b) small island, and c) large island techniques, are considered. To bridge a gap in the ossicular chain, various types of the middle-ear prostheses are available for all degrees of destruction [1]. In our paper, total ossicular replacement prosthesis (TORP) and partial ossicular replacement prosthesis (PORP) made from titanium or gold are considered. TORPs are used in the case of a completely destroyed ossicular chain and placed between the stapes footplate and the reconstructed TM. In the case of an intact stapes, a PORP is used and placed between the stapes head and the TM. Depending on the TM reconstruction technique, various mathematical models of the RME are proposed. In the case of the cartilage plate techniques, the restored ТМ is modeled as close to a circular soft plate [2], and in the case of the large island or small island techniques, the ТМ is represented as a soft two-layered plate or a system of co-centrically joint one- and two-layered plates [3]. If the stapes is still intact and mobile, the anisotropic stiffness of the stapes annular ligament is represented by a fully occupied stiffness matrix. Otherwise, in the case when the stapes is fixed and its footplate is subjected to fenestration, the stiffness of the round window is taken into account in both static and dynamic models. The influence of the geometrical dimensions of the prosthesis and its position on stresses in the TM and the annular ligament as well as on eigen-frequencies of vibrating system of the RME are studied. Analysis of the calculations performed allows one to conclude that in order to exclude possible luxation of the stapes or avoid high pretensions and the significant shift of the eigen-frequency
